 The Buffalo Therapeutic Riding Center New Student Application

Please fill out the information completely. If you are not requesting sponsorship,
you may leave that page out. The referral form must be filled out by the child’s physician
or therapist. Be advised that for safety reasons we can only accept students between the
ages of 6 and 16 and cannot exceed 150 pounds.

Please be advised that returning this application does not guarantee your
acceptance into the program. The Center’s Board of Directors reviews all applications
and has the right to deny acceptance to anyone whose disability may require services
beyond the expertise of the Center’s instructors.
